Your Appointment
1. Please bring photo identification
2. Bring a copy of your insurance card
3. If you are being referred by another physician, please bring a copy of your referral
4. Any copies of medical testing (e.g. ultrasound results, ER visits or testing done as a result of your referral)
5. Your co-payment is expected at the time of your visit (We accept cash, check, Mastercard and Visa)
Billing & Insurance Information
We accept most insurance coverage. For insurance holders that require a referral, you must contact your primary physician and inform the referral clerk that you have been referred to Women's Health Specialists.
Co-payments are due at the time of your appointment. These payments can be made with cash, or by credit card. Sorry, no checks are accepted.
To Cancel an Appointment:
Please telephone our office during business hours at least 24 hours prior to your appointment.
